Even with the advances in intraocular lens technology and the growing diversity of refractive surgery techniques, the role of contact lenses in ophthalmic practice has only increased. This is due in part to the great strides in materials, technology, expanding applications both refractive and therapeutic for contact lenses, and the clear recognition that contact lenses will always be an important tool for the ophthalmologist. With the fitting of contact lenses as a medical art, requiring a thorough understanding of anatomy, physiology and optics of the eye, this practice is formulaic only in part.

Four different types of soft contact lenses were randomly fitted to both eyes of each subject on two separate visits. UL-OCT imaged each lens in vitro and the ocular surface of a physical model eye. Mismatches between the lens and the ocular surface were simulated based on UL-OCT images of the in vitro lenses and the model eye. The existence of tear film gaps and touching points were predicted in the simulation.

Purpose : To evaluate the visual performance and optical quality of a standardized asymmetric soft contact lens SCL used for correction of higher-order aberrations HOAs in eyes with keratoconus. Methods : We included 30 eyes 26 patients with keratoconus average K: The patients were subjected to corneal tomography, aberrometry, measurements of manifest refraction and visual acuity VA , and visual analog scale VAS assessments. The study lenses were made using a molding method and consisted of six standardized types, in which an asymmetric power distribution of approximately 2 to 12 D 2-D step was used to correct HOAs. The lens type suitable for each eye was selected based on the corneal tomography and aberrometry data. The on-eye performance of the lens was evaluated using aberrometry 4-mm pupil , over refraction, VA, and VAS. Conclusions : A standardized asymmetric SCL can reduce HOAs and improve vision quality when compared with spectacles in patients with keratoconus who wear rigid gas-permeable lenses.
